Please note - The presenter is sent all the completed paperwork so you only need to print out the invoice or the evaluation forms if the presenter has forgotten or lost the originals.

The artist and the school must sign the BOCES invoice, the school must complete a School Evaluation Form, and the artist must complete an Artist Evaluation Form.
